As a Case Manager / Care Navigator, you will:
*Completes comprehensive psychosocial assessment and review of clinical needs of patients
with focus on patient's requirements as they transition to the next level of care
*Collaborate with physicians, social workers and other disciplines involved with care of the patient to foster a coordinated approach to discharge planning and preventing delays in patient care; communicates barriers to leadership for resolution and trending
*Identify services and resources available in the community and assists with patient connection to these services
*Participate in Interdisciplinary Rounds and other patient care conferences
*Collaborate with Utilization Review staff
*Responsible for reviewing medical records for clinical criteria, appropriate level of care and compliance with standards
*Able to review medical records for clinical criteria, appropriate level of care and compliance with standards

Qualfications Include:
* Maryland Registered Nurse license
*Bachelor's of Science in Nursing
*Time Management Efficient and accurate reviews
*Knowledge of disease management
*Prior case management or discharge planning
experience is preferred.
*Prior experience with Cerner
EMR and All Scripts is preferred
*Knowledge of Interqual/MCG preferred

Work Schedule:

Day Shift, M-F, 8:30am-5pm, and rotating weekends
